To secure your place in the program, please complete the following registration steps. Our Eir Centre team will be available to support you throughout the process.

• Register: Complete the form above or call the Eir Centre at (226) 289-6644.

• Create your Jane account: Set up your account through Jane Online Booking, where you can register, make payments, and book your required medical assessment.

• Complete your intake form: Once registered, you will receive an intake form by email to complete before the program begins.

• Submit your deposit: A $500 deposit is required to reserve your place in the program.

• Complete your medical assessment: **This is only necessary if you are participating in the Ketamine sessions** Attend your required medical assessment to ensure the program is a safe and appropriate fit for you.

• Submit your remaining balance: The balance will be due prior to the October 13, 2026 start date.

• Cancellation Policy: Participants who cancel prior to September 22, 2026 will receive a full refund of the $995 program fee. Cancellations made on or after September 29, 2026 are non-refundable; however, participants may be eligible to apply their payment as a credit toward a future Eir Centre program, at the discretion of the Eir Centre.
